Interim Head of Integrated Discharge

Practicus is partnering with a hospital based in the East of England to the interim Head of Integrated Discharge / Programme Lead This is a fantastic opportunity for you to make a real difference to these organisations'' bed capacity, discharge and DTOC performance whilst tackling stranded and super stranced patient groups.

This is a hands on role where you will be involved in both the reactive day to day management and longer term planning. This is a business critical role and will make a real difference to the provision of care to patients across the region.

You will take the lead on working with the acute trust, community provider and social care provider to deliver truly integrated discharge processes, improve LOS and reduce the significant number delayed discharge cases.

Your ability to engage with clinical teams and find sustainable long term solutions to this problem. The trust is currently one of the most challenged in the country, so this a chance for you to be part of this business critical delivery.

You will have gravitas and be a credible hard working manager, who has a proven track record in a similar position. The team are working hard and with your leadership, ideas and support they are confident in their ability to deliver sustainable improvements to patient care.
